Begin DTC Application
In order for the Medical Director to complete the medical portion of the Disability Tax Credit application you must have been diagnosed at the Adult ADHD Centre. For more information about our ADHD Assessments visit here.
The DTC application forms are completed and signed by the Medical Director, Dr. Gurdeep Parhar MD CCFP CCBOM CIME. Dr. Parhar has certification as a Disability Medicine and Occupational Medicine Physician with 30 years of experience assisting patients with a wide range of disabilities.
Disability Tax Credit (DTC) Application Process

Make payment online for $350.00 which is the Adult ADHD Centre fee to complete the Disability Tax Credit Application Form.

You will be contacted by email within 2-3 days with further information.

The Disability Tax Credit Application, completed and signed by the Medical Director, will be emailed to you. The Medical Director only completes the pdf/paper version of the Disability Tax Credit application form, not the online application form.

After you complete your section of the Disability Tax Credit Application, you may submit it to your accountant or directly to the Canada Revenue Agency (CRA).
